Before your appointment

  • PLEASE notify the doctor if you are experiencing covid like symptoms or have had recent known exposure to someone diagnosed with COVID-19.

  • Try to find a private, quiet, well-lit area for your visit to help you and your doctor see and hear each other better.

  • A room with lots of space can be beneficial if a doctor is assessing movement.

  • Check any emails from your health-care provider. There may be updates or helpful tips for the appointment.

  • If a family member or caregiver is joining the appointment, make sure they know the date and time.

  • Be available 15 minutes before and 30 minutes after your appointment time, as the doctor could be running behind.

  • Be aware doctors may call from an unknown or blocked phone number. Please Answer the call.

During your appointment

  • Inform the doctor of any medical problems or illnesses you may have, as certain medications, vitamins, and/or minerals may be contraindicated for you.

  • Have a list of all medications recently taken within the last 6 months and currently taking. Having the actual medication bottles is preferable.

  • Pharmacy information: addresses and phone number, in case the doctor needs to contact the pharmacist.

  • Pen and paper for any notes you would like to take during the appointment.

  • Be prepared to show any wounds or rashes for the doctor to assess.
